Urine Collection Instructions

Follow these simple steps to complete your urine sample collection:

Step 1: Prepare Your Kit

Remove all contents from your collection kit. You should have written instructions, collection materials, and a welcome card with QR code.

Step 2: Activate Your Kit

1. Scan the QR code on the welcome card
2. Enter your first name, last name, and date of birth
3. Your kit is now activated and ready for use

Step 3: Collect Your Sample

1. Collect urine in the provided collection cup
2. Unscrew the cap from the urine tube
3. Fill the tube to the 15 milliliter line
4. Screw the cap on tightly
5. Dispose of the collection cup in the trash

Step 4: Label and Package

1. Label the tube with the date and time of collection
2. Place the tube inside the provided bag
3. Place the bag containing the tube into the biohazard bag
4. Place the biohazard bag in the prepaid FedEx return envelope

Step 5: Ship Your Sample

Important: Ship within 12 hours of collection
