How to Create a New Patient Portal Account

  1. On the navigation bar, hover over Patients and click New Patients from the dropdown menu.

  2. Click Input Your Information under the “New Patient Registration” section.

  3. Fill in all contact information

  4. Create Username/Password

  5. Click Sign Me Up!
